Individual health is a core focus in the care continuum, and a strategy for enhancing life satisfaction. The goal of our Health Services Department is to provide community based support to individuals, fostering health, safety, welfare, and overall quality of life. At Keystone, we focus our efforts on providing preventative health care services as a strategy for basic health maintenance.
Keystone Community Resources has 50 years of experience in providing community based services. Through our rich experiences, we have developed long-standing relationships with an assortment of physicians and health care providers. Our consumers require the services of physicians in a wide variety of disciplines including general practice, psychiatry, dentistry, audiology, ophthalmology, podiatry, and gynecology. Our relationships foster success in accessing quality care, and bolster the quality of the health services they receive. We also work closely with the regional Health Care Quality Unit (HCQU) to assure that our services meet stringent standards. The HCQU provides both support and oversight in our provision of health services. They assist in delivery of modern training modules and best practice approaches to specific health care needs. Additionally, they provide specific case oversight, analysis and support.
Keystone Community Resources and the Health Services Department provide Supportive Nursing services to each of our programs. Community based nurses are available to support individuals 365 days a year. Our team of professionals provide an assortment of services to support individuals in our programs, and provide another layer of oversight in the continuum of care.
Individuals are assigned a nurse responsible for coordinating healthcare needs and to assist with any additional medical support. As a member of the Interdisciplinary team of professionals, our nurses work closely with community homes and day program sites to assure all aspects of the individual’s health needs are met, facilitating optimal care.

In addition to the broad array of community based supports,
the Health Services Department maintains a centralized, community-based office,
equipped to deal with routine and basic emergency medical needs. The office has a fully equipped examination
room utilized by physicians during on-site visits and examinations. This
resource provides flexibility in providing services and increases the quality
and satisfaction of our consumer’s care.
